q Complete 24 hours of academic instruction
q Take and pass assessment exam. Register at the SOS
q Complete registration sheet. This can be found at the State Registry
q Obtain fingerprints from local police post on an FBI fingerprint card (FD258).
q Pack and mail all items required
q Certificate of academic completion
q Proof of assessment passing score
q If you must support disclosure questions, send supporting documentation
q Fingerprint card (FD258)
q Check for $32.25 for FBI background check. Make checks payable to Indiana Secretary of State.
